Vascularized Joint Transfer from Proximal Interphalangeal Joint to Metacarpophalangeal Joint- A Case Report

帶血管莖近端指間關節至掌指關節轉移術-一病例報告

摘要


帶血管莖關節轉移術,可作為指關節損傷之重建。我們報告一例22歲男性利用帶血管莖關節轉移術,將右中指近端指間關節轉移至掌指關節之病例。病患因右手壓傷,接受了第2、3指重接術;第4、5指截肢術。術後之右中指掌指關節為連枷指。病患要求在不做其他部位犧牲的條件下,增加該病患手之功能。考慮掌指關節之重要性,我們幫病人實行帶血管莖近端指間關節至掌指關節轉移術;近端指間關節固定術。術後10個月追蹤發現掌指關節之運動範圍約為30至60度。關節穩定度良好。

Vascularized joint transfer has been used for the reconstruction of traumatized finger joints. We report a vascularized joint transfer from the proximal interphalangeal (PIP) joint to the metacarpophalangeal (MP) joint of right middle finger (RMF) in a 22-year-old male. The patient suffered from a crushing injury of his right hand and received replantation of the index and middle fingers and revision amputation of the ring and little fingers. The MP joint of middle finger was unstable because of pseudoarthrosis. The patient asked for improvement of his hand function without sacrifice of any donor site. Consider the importance of MP joint over PIP joint, vascularized PIP joint transfer to the MP joint and arthrodesis of the PIP joint were performed. Post-operative 10-month follow up showed the range of motion of the MP joint was 30 to 60 degrees and the middle finger become stable and helpful.
