Homodigital Vascularized Joint Transfer of Distal Interphalangeal Joint to Proximal Interphalangeal Joint-A Case Report

摘要


在年輕且生活活躍的病人當中,對於已損壞手指的近端指間關節重建是一個困難的挑戰。對於同一雙手指,帶有血管莖的遠端指間關節是一個相類似的關節,這可以用於壞損的關節置換並且它的損失是可以被接受的。我們藉此報告一位20歲男性因爲右小指陳舊性外傷而造成他的近端指間關節炎合併完全強直。藉由帶血管莖遠端指間關節至近端指間關節轉移術,右手小指重建的近端指間關節活動範圍可以從0度到65度。藉由這樣的手術,已經受到傷害的手指可以充當自己的供應者並且沒有其他手指或者腳趾的犧牲,也由於它的結果很好,這種手術目前是我們治療這類年輕病人的方法。

Reconstruction of the damaged proximal interphalangeal (PIP) joint is a difficult task, especially in the young active patient. The distal interphalangeal (PIP) joint of the same digit, used as an island vascularized joint, can be useful for this potential transfer and its loss is well tolerated. We hereby report a 20-year-old male sustained an old trauma with stiff PIP joint of his right little finger. Following homodigital DIP to PIP joint transfer, the active motion of the PIP joint was restored from 0 to 65 degrees. With this procedure, the traumatized digit serves as its own donor and no other digit or toe is compromised, and because of its excellent result, it is currently our preferred method for young patient.
