摘要

原發性肛門黑色素瘤臨床上並不多見，因其臨床症狀常常為肛門出血、疼痛及腫瘤，因此常常被誤以為良性腫瘤例如痔瘡或息肉。目前對於此疾病並無一致的治療方式，且常延誤診斷及缺少有效的系統性療法，因此其預後很差。目前對於原發性肛門黑色素瘤的手術治療主要有廣泛性的局部切除或肛門會陰切除術，文獻的統計兩者的五年存活率無顯著不同，因此報告此一原發性肛門黑色素瘤病例接受廣泛性的局部切除及植皮手術，病人目前已追蹤二年無局部復發或遠處轉移。本文將回顧及討論此腫瘤的臨床特性、病理及治療。

Anorectal melanoma is a very rare disease with a high mortality rate. The patient usually presents with a sensation of mass and rectal bleeding that is easily mistaken for a benign hemorrhoid or rectal polyp. Distal metastasis is common due to the delayed diagnosis and aggressiveness of the disease. We herein describe a case of a 72-year-old woman who presented with an anorectal mass. The patient received an excisional biopsy which was proved to be a rare anorectal melanoma. The authors subsequently performed a wide local excision with a 3 cm safety margin and skin graft two weeks following the initial diagnosis. The patient has been well without any evidence of recurrence after two-years of follow-up.
