血液透析病患運動訓練之安全性監測及主觀評量

Exercise Training Safety and Subjective Evaluation in Hemodialysis Patients

摘要


本研究旨在探討血液透析病患運動訓練之安全性監測及主觀評量。採單組前後測研究設計方法,於南部某醫學中心以立意取樣選取20位血液透析病患在非洗腎期進行每週三次,每次30-50分鐘,爲期八週踩直立式腳踏車之運動訓練。在每次運動訓練前、中、後及運動訓練八週前後比較收縮壓、舒張壓、心跳、脈搏血氧飽和度、自覺費力程度與心電圖之變化並在運動訓練八週後透過開放性問題來評量個案主觀感受之成效。研究結果發現個案於每次運動訓練前、中、後的收縮壓、舒張壓、心跳、脈博血氧飽和度及自覺費力程度皆在正常範圍,心電圖呈現有良性心室早期收縮情形。並在運動訓練八週前後的心跳及自覺費力程度具有顯著下降。且在運動訓練八週後有85%個案表示在走路及爬樓梯時大腿較有力氣感。由本研究結果顯示血液透析病患之運動訓練是安全及有效的,可提供透析單位在未來推行運動訓練之參考。

The purposes of this study were to explore the safety of exercise training and to evaluate subjective feeling in hemodialysis patients. A single pre- and post-test was used to collect data from participants at a medical center in southern Taiwan. Purposive sampling was used to recruit twenty hemodialysis patients who received stationary bicycle exercise training during off-dialysis periods. The exercise training program consisted of 30 to 50 minutes of exercise three times a week for 8 weeks. Blood pressure, heart rate, O2 saturation, rate of perceived exertion, and electrocardiogram (EKG) were checked prior to the start of exercise, during warm up, during exercise, immediately after exercise, and again 5~10 minutes post-exercise. The subjective feeling was evaluated after 8 weeks of training using an open-ended questionnaire. The results showed that systolic and distolic blood pressure, heart rate, O2 saturation, and rate of perceived exertion were within normal range for acute effects, However, the EKO showed premature ventricular contraction response in some patients. Resting heart rate and rate of perceived exertion were decreased for chronic effects. For subjective evaluation, 85% of the subjects reported increased leg muscle strength during walking and climbing stairs. The findings imply that the exercise training program for hemodialysis patients was safe and effective and worthy of implementation in dialysis units.
