一位接受膝上截肢青少年手術後之護理經驗

Nursing Experience of an Adolescent with Above-Knee Amputation Surgery

摘要


本文是描述一位右膝上截肢青少年手術後的護理經驗,筆者自2005年6月14日至7月9日護理期間,運用Gordon十一項健康型態功能評估作爲指引,收集資料並確立問題,發現個案有疼痛、身體心像紊亂、身體活動功能障礙及知識缺失等健康問題。照顧過程中藉由放鬆技巧減緩疼痛,提供主動關懷,情緒支持、宣洩壓抑的情緒,鼓勵正視身體的改變,使其面對截肢的事實並與家屬共同協助執行復健運動,進而主動學習助行器使用,逐漸減少對家屬之依賴,並衛教出院後對截肢之注意事項,使個案出院能重新適應日常生活的自我照顧,筆者期望藉此護理經驗,能提供臨床護理人員照顧青少年截肢患者之參考。

This article described the nursing experience of an adolescent with a right above-knee amputation surgery. The author collected the data from June 14 to July 9, 2005 by using Gorden's 11 functional health assessment tool and identified problems of pain, body image disturbance, physical mobility impairment, and knowledge deficit. The caring process included reducing patient's pain by the relaxing skill, providing active sympathy and emotional supports, revealing suppressed emotion, encouraging the subject facing the reality a change of the body, and helping him and family members perform rehabilitation together. In addition, self-care skills such as using assisted device to reduce the dependence and care about amputation after discharge were also provided to assist the patient adjusting for his daily life. The nursing experience provided an useful reference for caring adolescent patients with amputation.
