摘要

本文描述一位因女友情變改嫁他人，加上工作不順心以割腕及服用安眠藥自殺個案之護理經驗。筆者於2008年4月28日至2008年6月10日護理期間，以觀察、身體評估、直接護理及筆談等方式進行資料收集，發現個案有呼吸道清除功能失效、無望感、無效性因應能力等健康問題。運用關懷與提供疾病進展之相關訊息來建立互信，以肢體接觸、適當保證與提供一個尊重隱私的環境來增加安全感，教導宣洩其負向情緒的技巧來緩解壓力，引導其說出自身優點及利用正向思考來肯定其價值，邀請家屬參與照護並表達對彼此情感的依戀，漸漸地個案願意敞開心房，重新適應生活並對未來重新懷抱希望。希望藉此分享情變自殺患者的照護經驗，在加護病房除了滿足生理需求及維持生命，更應該撫慰受創心靈，陪伴其渡過情緒的低潮。

This article described the nursing experience of a suicidal patient whose ex-girlfriend was going to marry his best friend. The patient developed workplace issues before overdosing on drugs and cutting his wrist. The author collected data through observation, physical assessment, direct nursing, interviews, and written communications from April 28, 2008 to June 10, 2008. The author confirmed the diagnosis and found that the patient had physical problems, such as irregular breathing patterns. The patient also had some related mental health problems, including a feeling of hopelessness and ineffective coping skills. The author established a mutually caring positive relationship with the patient and developed trust in the nursing process, provided disease-related information assistance, used tactile therapy, and reassured the patient while providing a safe environment and respect of privacy. The patient was taught anger management techniques and the family was invited to join the treatment programs. Assistance was sought in receiving support from family members. The patient's ability to adjust to his current situation improved and he looked forward to the future. This experience can benefit nursing professionals with a greater understanding of suicidal patient care in the ICU; not only in focusing on physical problems, but also in helping the patient pass quickly through the emotional and grieving process.
