照顧一位燒炭自殺顏面燒傷患者之護理經驗

摘要

燒炭自殺患者經常導致燒傷，本文為照顧一位有憂鬱症病史患者，此次係在家服用安眠藥後燒炭自殺，造成右臉嚴重燒傷之護理經驗。護理期間自2010年2月28日至4月28日，筆者於護理期間以觀察、會談、身體評估及查閱病歷等方式蒐集資料，以Gordon十一項功能性健康型態為評估指引，發現主要護理問題為低效性呼吸型態、身體心像紊亂及潛在危險性自殺。在燒傷加護病房期間監測氧合及安排呼吸運動，給予支持及陪伴，主動關懷，鼓勵說出外貌改變的感受，接受外貌改變及增強正向態度，引導正向情緒抒發，及增強生命價值，進而出院返家重返社會，希望藉由此經驗提供臨床護理人員對於顏面受傷患者面對身體心像改變照護經驗之參考。

This case report described the intensive care nursing experience of a depressed patient with right facial burn caused by charcoal-burning suicide following taking hypnotics. The caring period was from February 28, 2010 to April 28, 2010. The Gordon's functional health assessment guide was used to assess nursing problems through observation, interview, physical examination, and chart review. The nursing problems identified included breathing pattern, ineffective, disturbance body image, and risk for suicide. Intensive care interventions included monitoring oximeter and arranging breathing exercise, providing support and companionship and active caring, encouraging to express perception of appearance, leading positive emotional express, and enhancing recognition of value of life. This experience provided nurses with a reference for caring similar patients in the future.
