一位青少年動靜脈血管畸形併腦出血之護理經驗

Nursing Experience of Caring An Adolescent with Arteriorvenous Malformation and Brain Hemorrhage

摘要


本文探討一位17歲少年因動靜脈畸形併腦出血術後恢復期之護理經驗,筆者於2017年8月10日至8月19日以觀察、身體評估、會談等技巧進行資料收集。採Gordon十一項健康功能型態作為評估指引。確立有:1.身體活動功能障礙。2.自我照顧能力缺失。3.身體心像紊亂等護理問題。過程提供復健方案,指導自我更衣沐浴方式,提升自我照顧能力並預防跌倒,同時更引導其正向接受外觀改變。未來建議轉介系統整合校園健康中心,持續輔導訪視及追蹤,協助校園適應及生活品質。盼藉此護理經驗分享,提供護理人員照護類似病人之參考。

This article described the nursing experience of caring a 17-year-old boy during the recovery period after experiencing cerebral hemorrhage resulting from arteriovenous malformation. The caring period was from August 10 to 19, 2017. Data were collected through observation, physical assessment, and conversation. Gordon's functional health patterns were used as assessment guidelines. The nursing problems were identified as physical activity dysfunction, self-care deficit, and body image disturbance. In the nursing care process, a rehabilitation program and an intervention program of teaching patient change clothes and take a bath by himself was implemented. Finally, self-care ability of patient was improved and the risk of fall was reduced. Patient could finally accept the appearance change positively. It is suggested that a referral system can integrate the campus health center to achieve continuous counseling and follow-up visits and improve the patient's quality of life.
