一位學齡期動靜脈畸形個案跨國求醫之照護經驗

本文是一位寮國12歲女性腿部陰唇動靜脈畸形，伴隨右坐骨潰瘍傷口，造成身心衝擊並跨國來台求醫之護理經驗。護理期間自2019年10月30日至2020年1月31日，運用Gordon十一項健康模式評估，並藉由觀察、會談、傾聽及身體評估方式，確立有組織完整性受損、急性疼痛、身體心像紊亂等健康問題。護理期間醫療團隊透過翻譯員、圖卡及網路翻譯達到雙向溝通，並藉由右陰唇血管栓塞及右陰唇腫塊切除手術改善雙腿間腫塊之異物感，右坐骨潰瘍傷口以清創手術，加上銀離子敷料照護，促進傷口癒合並緩解疼痛。期望分享國際醫療罕見個案之照護經驗。

This study reported a case of a 12-year-old girl from Laos with a leg and labial arteriovenous malformation accompanied by a wound on the right ischial ulcer, which impacted her physical and psychological health. She sought medical attention via a "Trans-national" or cross- national" service from October 31, 2019, to January 31, 2020. The author assessed the client using Gordon's 11 health functions evaluation tool. Data were collected through observation, interviews, and physical examination. The chief problems were identified, which included the integrity of impaired tissues, acute pain, and body image disturbance. During hospitalization, the medical team achieved two-way communication through translators, graphics cards and network translation, then the child underwent right labia vascular embolization and right labia lump resection. After successful surgery, the lump between the legs disappeared, and wound debridement was performed along with ion dressing care. The wound healed gradually and pain was eased. We wish to share this experience of a rare case in international medical care.
