本文是探討一位學齡期兒童接受扁平足矯正手術之恢復室護理經驗。筆者於2018年02月05日16:10~18:25共計照護2小時15分鐘。期間以觀察、評估、直接照護方式收集資料,確立個案有疼痛、焦慮及案母為主要照顧者角色緊張等三項護理問題。護理期間筆者以增加探視個案頻率並運用傾聽、陪伴、同理心,建立良好信任的護病關係,鼓勵個案及案母表達其情緒,根據學齡期兒童發展特性,以兒童溝通技巧與遊戲,並依個案的喜好的事物來轉移注意力、緩解個案疼痛不適感以及對醫療陌生環境不安焦慮的情緒,並主動提供案母術後相關照護資訊、鼓勵參與個案換藥過程及教導上網搜尋可利用資源,提升案母的照護能力,降低其不安與擔憂,使個案獲得更完整的照護。建議藉由在床旁營造兒童的遊戲氛圍、設立遊戲行動櫃,放置各年齡層書本或是醫療模型玩具等來轉移兒童注意力,以降低兒童術後的焦慮與不安。
This report described the experience of caring for a school child in the recovery room after receiving a flatfoot reconstruction operation. The researcher conducted 2 hours and 15 minutes of care from 4:10pm to 6:25pm on February 5, 2018. During this period, observation, evaluation, and direct care were used to collect data. The patient was identified to have three problems requiring care: pain, nervousness, and caregiver role strain. During the care period, the researcher visited the patient with increased frequency and built up trust in an excellent nurse-patient relationship, with listening, accompanying and empathy. The patient and the patient's parents were encouraged to express their emotions. According to the characteristics of school children's development, children's communication skills and games were used. In addition, according to the patient's favorites, her attention was redirected to reduce the patient's pain, discomfort, and the stress caused by an unfamiliar environment. Care information was provided to the patient's parents actively, encouraging the patient to participate in the dressing change process, and instructing them to search for useful information on the internet to promote the ability of the patient's parents in caring. After reducing the patient's anxiety and worry, we provide the patient with more complete care. It is suggested to construct an atmosphere of games around the sickbed to establish a portable game cabinet with books for different age groups or medical models and toys to distract the children's attention to reduce their stress and anxiety in postoperative hours.
