健康照護矩陣於藥事服務之適用性評估

摘要

目的：健康照護矩陣是一項兼具醫療品質指標與核心能力的評估工具，用於檢視藥事服務，能提供多面向的思考模式，有助提升服務品質與臨床技能。本文目的在介紹藥師運用健康照護矩陣檢討臨床藥事服務品質及改善措施。方法：搜尋文獻與參考本院醫學教育資料，並徵得原著者同意，制訂藥師版健康照護矩陣評估表。接著選定藥師進行宣導，用於檢視處方建議的品質，其後針對健康照護矩陣的適用性進行問卷調查。結果：所修訂的健康照護矩陣由六大品質指標（安全醫療、及時服務、有用處置、效率工作、平等就醫、以病人為中心）與藥師七大核心能力（病人照護、醫療知識、實證醫學／資訊學、團隊系統考量、人際溝通、專業素養、學習與改善）組成，於2011年8月16日至2012年6月25日共有18位藥師完成25份矩陣，指標達成率以「平等就醫」（100%）最高，「及時服務」（56%）最低。問卷調查顯示超過80%的藥師認為矩陣有助發現可精進之處，並經由學習改善提升服務品質與專業能力，對日後業務有所助益，並可續用於檢視處方建議品質。結論：運用健康照護矩陣進行自我檢討與團體討論，可讓與會者有更多機會瞭解實務注意事項，因此以健康照護矩陣做為今後提高藥事服務品質與核心能力的訓練方式是具有高度價值。

Objective: Healthcare matrix is a tool widely adopted in advancing the core competency of healthcare care professionals and conducting medical quality training. We implemented a preliminary study to discuss the healthcare matrix application in pharmaceutical care.Methods: We modified the core competencies to compatible with which announced by TSHP in the year 2010. We obtained consent from the healthcare matrix original author, Bingham and Quinn. We investigated the applicability of modified healthcare matrix by questionnaire.Results: The modified healthcare matrix linked to the 7 pharmacist core competencies (which contain of patient care, pharmaceutical knowledge, evidence-based practice and informatics, system-based practice, interpersonal and communication skills, professionalism, and practice-based learning and improvement), and the 6 IOM dimensions of quality (which contain safe, timely, effective, efficient, equitable, and patientcentered) as a tool to improve healthcare. Although there was more time consumed to complete healthcare matrix, our pharmacists still give positive feedback and willing to use this tool.Conclusion: Self-assessment and discussion for pharmaceutical care with the healthcare matrix may provide participants more opportunities understanding practical concerns. Therefore, it is valuable for instruction of the matrix in terms of important and core competencies.
