摘要

Merkel細胞癌是一種罕見的原發性神經內分泌的皮膚腫瘤。它好發於老年人的頭頸部，經常會有局部復發以及轉移的情形發生，且其死亡率則可達到30%。Merkel細胞癌與其他種類的皮膚腫瘤以及血液方面的腫瘤同時存在同一個病人的身上的比例相當高，大約佔25-34%，但是Merkel細胞癌與表皮細胞癌並存在同一個皮膚病灶內則是非常罕見。在此我們提出一個病例報告，其皮膚上的一處病灶同時存在有Merkel細胞癌與及鱗狀細胞癌。根據以往的報告，合併此兩種腫瘤的局部復發率遠高於單獨存在的Merkel細胞癌。很遺憾的，此病人拒絕接受手術切除，且未回到本院追蹤。

Merkel cell carcinoma is a rare primary neuroendocrine skin tumor that usually arises in the head and neck of elderly patients. Local recurrence and metastasis are common and the mortality rate has been reported to be as high as 30%. There is a high incidence (25-34%) of coexistence with second neoplasm, including other skin tumors and hematological malignancies in patients with Merkel cell carcinoma. However, mixed Merkel cell and squamous cell carcinoma in the same lesion is very rare. Herein we report a 89-year-old women with such a tumor. According to the previous studies, the local recurrence rate of the mixed tumor is much higher than that of Merkel cell carcinoma occurred alone. Regretfully, patient refused complete excision and was lost to the follow-up.
