Interstitial Granulomatous Dermatitis-A Case Report Associated with Rheumatoid Arthritis
間質肉芽腫性皮膚炎合併類風濕性關節炎－病例報告
摘要
間質肉芽腫性皮膚炎是一種由Ackerman等人於1993年提出的罕見疾病。我們在此提出一個長期罹患風濕性關節炎的五十七歲女性，在其臀部與大腿內側出現對稱性的紫紅色浸潤性丘疹與結節約三個多月。在病理下可發現有柵欄狀排列的組織球圍繞著變性的膠原纖維，而浸潤的發炎細胞主要由淋巴球、組織球與散在性的嗜伊紅球至後續的真皮層纖維化。有些組織球有非典型化的表現。由臨床與病理表現，我們診斷這位病患為間質肉芽腫性皮膚炎。

Interstitial granulomatous dermatitis is a rare entity first described by Ackerman et al. in 1993. We described a 57-year-old female, with a long-standing history of rheumatoid arthritis, presenting with symmetric violaceous to erythematous indurated papules and nodules on her buttock and medial thighs arranging in an arciform fashion for more than three months. Pathologically, it showed palisaded granulomas containing degenerated collagen, predominantly lymphohistiocytic infiltration with scattered eosinophils leading to dermal fibrosis. Some atypical histiocytes were also present. Combining the clinical and pathological pictures, interstitial granulomatous dermatitis was diagnosed.
