The use of fibular osteoseptocutaneous flap combined with dental implants for mandibular reconstruction, improves not only the patient’s esthetic profile but also masticatory function. The disadvantages of this flap are increased thickness, mobility of oral lining soft tissue and lack of vestibular depth for conventioned prosthetic fabrication. The use of keratinized palatal mucosa graft and acrylic stent design are good means to overcome these problems. This technique is presented in the two cases in this report.
