Predictable reconstruction of the lost periodontal structure devastated by periodontal diseases is the ultimate goal of periodontal therapy. The use of guided tissue regeneration and osseous grafting with demineralized freeze-dried bone allograft results in more predictable periodontal regeneration. Furthermore, the surgical regeneration may be able to restore the supporting structures of the diseased tooth so as to convert its questionable or poor prognosis to a better one. This report presents three advanced cases with poor or hopeless prognosis of periodontally diseased teeth. With combination of root canal treatment and periodontal regeneration, the prognosis of these teeth are improved. With development of new concept and novel technology, the periodontally poor or hopeless teeth may be given a more favorable prognosis in the future.
