Use of Periodontal Regeneration and Root Canal Therapy for the Treatment of Periodontally Poor or Hopeless Teeth: Report of Three Cases

摘要

牙周治療的最終目的，是可預期的重建已受牙周疾病破壞的牙周組織。使用引導組織再生及脫鈣乾燥異體骨移植術，可以得到較可預期的牙周重建。另一方面，此手術性之重建亦可促進牙周支持構造之恢復，並改善其不良或有問題之預後。本報告提出三個嚴重牙周病之病例，牙齒之預後為不良或無望，但合併使用根管治療與牙周再生手術，其預後均得到明顯改善。在新觀念及嶄新科技的發展下，未來牙周嚴重破壞的牙齒或可得到較佳的預後。

Predictable reconstruction of the lost periodontal structure devastated by periodontal diseases is the ultimate goal of periodontal therapy. The use of guided tissue regeneration and osseous grafting with demineralized freeze-dried bone allograft results in more predictable periodontal regeneration. Furthermore, the surgical regeneration may be able to restore the supporting structures of the diseased tooth so as to convert its questionable or poor prognosis to a better one. This report presents three advanced cases with poor or hopeless prognosis of periodontally diseased teeth. With combination of root canal treatment and periodontal regeneration, the prognosis of these teeth are improved. With development of new concept and novel technology, the periodontally poor or hopeless teeth may be given a more favorable prognosis in the future.
