The treatment of complicated crown-root fracture is a challenge to dentists. Complicated crown-root fracture involving enamel, dentin & pulp that extend below both the gingival margin & the alveolar bone is difficult to restore. Invading the biologic width sets up a series of events through an inflammatory process that can lead to pocketing around the tooth by the body’s attempt to re-create this “width” through alveolar bone less. Therefore the treatment protocol is to expose the fracture line & to maintain the biologic width. There are various treatment options in management of complicated crown-root fracture, including crown lengthening, orthodontic extrusion, surgical extrusion & extraction. This case report describes an alternative treatment option, the intentionally rotated replantation, in management of an upper lateral incisor with complicated crown-root fracture.
