An impacted dilacerated incisor will result in tilting of the neighboring teeth, midline deviation, and space deficiency in the dental arch. If the dilaceration of root is below 87.8°, and the inclination of the impacted tooth crown axis is between 180° to-60°, surgical exposure followed by orthodontic traction is recommended to treat the impacted tooth. For maintaining good periodontal health, we can not exposure the cementoenamel junction of the impacted tooth when doing the surgical procedure. For preserving adequate attached gingiva, surgical window technique for labially impacted teeth is not use. This case report describes the successful treatment of an impacted dilacerated incisor using surgical exposure using apically positioned flap and orthodontic traction. The tooth with adequate attached gingiva reveals good periodontal health after two-year follow-up.
