This is a case of generalized chronic periodontitis with occlusal problems which include the loss of right posterior occlusion and supraeruption of the right upper posterior teeth. The patient is presented with a gummy smile and an improper crown ratio which impairs esthetics. Through basic periodontal treatment, the periodontal tissue health improved significantly. A series of further treatment, implant surgery, adjunctive orthodontic treatment, and surgical crown lengthening, was done to restore function and esthetics for the patient. Dental implants not only restore the missing teeth, but also serve as an efficient anchorage in case orthodontic treatment is needed. Excess gum display is also a concern for esthetics. Crown lengthening can alter the width-to-height ratio, and serve as a measure to improve the patient's gummy smile. A thorough assessment of the cause of a gummy smile is mandatory prior to surgery to establish a biological requirement for periodontal health.
