摘要

在前牙區的局部骨缺損，對於贗復科及美容牙醫學而言已演變為重要的議題。從贗復牙醫學及美容牙醫學的觀點越來越注重笑容時的牙齦美學。本病例報告中，＃12和＃22因創傷性拔牙後而導致齒槽骨嚴重吸收。臨床上缺牙時骨頭吸收塞本上是第一類骨頭缺損（頰側部分向舌側方向吸）。治療這些骨缺損有很多方法。若以人工植牙為治療計畫時，引導骨再生術可以用來解決骨缺損的骨量不足的問題。以牙周病學來考量，皮下結締組織的移植對於美觀已有良好的改善。病人以美觀改善為主訴，皮下結締組織從上顎隆凸取下移植至＃12和＃22缺牙區，術後八個月回診，病人對美觀的改善感到滿意。

Local Alveolar ridge deformities in the anterior area presented a great concern during restorative and esthetic dentistry. Gingival esthetics to a smile has become a primary importance by esthetic and prosthodontic practitioners. In this case, the patient has Class Ⅰ (buccolingual) defect in tooth #12 and 22 due to traumatic extraction, the treatments of these defects can be treated with various approaches. If the case was prosthetic driven with possibility of dental implant placement, then guided bone regeneration will recover lost hard tissue in order to have enough bone for dental implants placement. If the case is a periodontal driven case, then usage of connective tissue graft to augment the site might be the way to choose. Esthetic was the main concern for the patient, connective tissue graft taken from the tuberosity area then grafted onto both #12 and #22 area. After 8 month follow-up, satisfactory esthetic result was presented using connective tissue graft.
