使用合成性再生膜進行引導骨再生術造成術後骨破壞:病例報告

摘要


當患者欲植牙區之骨量不足時,常需藉由引導骨再生術的輔助,以達到植牙所需之骨量,由於其結果的可預期性,比術式已成爲輔助植牙手術的常規術式之一。在引導骨再生術中,再生膜之使用佔有極重要之角色,市面上亦有許多不同材質、廠牌可供選擇。本病例報告爲三位在奇美醫學中心進行人工植牙手術之忠者,皆無全身性疾病及不良習慣,但在使用聚乳酸再生A莫及骨移植體進行引導骨再生術後,發生牙海腫脹、竇管、骨破壞等併發症。因再生膜的材質會影響術後組織的反應,根據此三病例之發現,推論聚乳酸再生膜可能潛藏較不利組織之反應特性,特此提出報告並探討造成此併發症與使用聚乳酸再生膜間可能之關聯性。

Guided bone regeneration (GBR) has been proved to he a suitable and predictable technique for promoting bone regeneration. A variety of synthetic derived GBR harriers have been clinically used to facilitate bone regeneration. These barriers may differ from composition and structure and may affect the outcomes of GBR. We present 3 consecutively treated patients, in whom GBR with polylactic acid (PLA) membranes were used to correct bony defect. All showed varied degree of inflammatory foreign-body reaction, and clinically exhibited swelling, sinus tract or osteolysis. It may he speculated that degradation products of the PLA device may adversely affect a regenerative process. According to these finding, patients undergoing GBR procedures with synthetic absorbable devices should he advised of the possible occurrence of such an adverse effect.
