摘要
咬合傷害是臨床上牙周病患者常見的問題，咬合傷害的控制對中度到重度牙周炎患者是不可或缺的一環。本文提出一全口慢性牙周炎合併咬合傷害之四十八歲男性病患的案例，以全口超音波清除牙結石、牙根整平術、翻瓣清創等方式對牙周發炎狀況進行控制，並以咬合調整合併補綴物重新製作方式進行治療。於半年觀察期後進行再評估時已可見良好之臨床結果。繼續追蹤一年後，整體牙周探測深度都在4釐米以內，放射線檢查也能發現原本骨下缺損處皆有骨充填現象。病人可以維持良好口腔衛生。探測出血指數及牙菌斑指數皆有降低。病患整體牙周健康獲得控制及改善。（台灣牙周醫誌17: 1-13, 2012）

Trauma from occlusion is commonly found in patients with periodontitis. In order to achieve better clinical outcome, control of trauma from occlusion is necessary in the cases of moderate to advanced periodontitis. This article will address the treatment course for a 48-year old male patient diagnosed with generalized chronic periodontitis with trauma from occlusion. Full mouth ultrasonic scaling, root planing and open flap debridement were performed to control periodontal inflammation. Further occlusal adjustment and re-fabrication of prosthesis were performed during active treatment phase. After 6 months, a re-evaluation was done and a good clinical result was obtained. At 1 year follow-up, the probing depths of all sites were within 4 mm. Bone- filling could also be found at the previous infrabony defect sites from radiographic examination. The patient also had maintained good oral hygiene. Both bleeding on probing and plaque index were decreased. Overall, the periodontal condition of this case was controlled and improved.
