The Survival and Success Rate in Maxillary Molar Region of Root Resections Compared to Single-Tooth Implants: A Narrative Review

牙根切除在處理根叉侵犯的牙齒或病變集中於多根牙的局部牙根時，一直是很重要的治療模式之一。這些局部病變包括牙周破壞、牙根斷裂、牙根齲齒、根管治療失敗或是牙周與根管合併病變等，都可以考慮使用牙根切除來治療。目前雖然關於牙根切除的成功率與存活率都有文獻記載，然而大部分的文獻討論到牙根切除的存活率時並沒有把上下顎的臼齒分開計算，這忽略了上下顎臼齒的形態與周遭解剖構造並不相同。另外要考慮的一點則是，隨著現今植牙治療的高成功率，牙根切除是否有可能仍是治療的選擇之一？因為上顎區域解剖構造的特殊性，本文獻回顧針對牙根切除術與臼齒區單顆植牙於上顎之存活率進行比較，並針對這兩種術式的適應症、手術與補綴方面等考量進行討論。

Root resectioning used to be one of the important treatment modalities when managing teeth with furcation involvement or teeth with untreatable pathosis that is limited to some of the roots in one tooth. These problems can include periodontal destruction, root fracture, root caries, endodontic failure or even a combination of these problems. The long-term success or survival rates of root resections have been well documented. However, most of the literature has discussed the survival rates of root resection without separating the maxillary molars from the mandibular molars; however, these teeth are quite different in both morphology and anatomical structure. Another factor that needs to be considered is that, because there is such a high success rate for dental implant therapy at present, is root resectioning still a viable optional treatment among the several treatment modalities available for solving furcation problems? This review article compares the survival rate of root resections with those of single implants and targets the maxillary molar region due to the special anatomical characteristics of the maxillary molars. The indications, surgical requirements, and restorative considerations of both treatment methods are discussed.
