Neglected Rupture of the Urinary Bladder in a Patient with Tonic-clonic Seizure and Mental Retardation

膀胱破裂並不多見，大都伴隨著嚴重的創傷。本文報告一例，可能因癲癇發作跌倒而導致膀胱破裂，然而病人因智障，無怯正確表達其身體不適，而被照顧者長期忽略，過了三個月才因癲癇控制不佳及頭部外傷而到醫院求診，此時身體無明顯外傷，但因為嚴重腹脹、腹水及血尿，經膀胱攝影及膀胱鏡檢查後，證實膀胱破裂病人接受緊急手術施行膀胱修補，於一個禮拜後痊癒出院。

Rupture of the urinary bladder is an uncommon condition and is usually caused by severe trauma. Neglected urinary bladder rupture in the absence of evident trauma history is rare. We report a patient with a history of mental retardation and epileptic seizures who presented to our hospital with facial laceration after recently falling due to seizure. No other obvious injuries were noted, however, we observed severe abdominal distension, ascites and hematuria. The patient's care giver mentioned that the patient had complained of abdominal pain and dysuria for the past 3 months but the symptoms had been ignored by hospital staff. A preliminary diagnosis of intraperitoneal rupture of the urinary bladder was made and a cystography and cystoscopy were conducted. Those procedures confirmed the diagnosis and the patient underwent urinary bladder repair. She recovered and was discharged 1 week later.
