迷你骨釘錨定在二級異常咬合及深咬的應用－病例報告

摘要

迷你植體錨定應用於各種狀況的矯正治療已有顯著的成效，包括雙顎前突、第二級或第三級咬合的改正、全牙弓擁擠的後退、後牙過度萌發、中線偏移、咬合平面傾斜、後牙錯咬的改正，都有極大的幫助。以住我們使用迷你骨釘主要用於最弦錨定後退(maximal retraction)，極少用於前牙深咬(deep bite)的改善，主要是因為迷你骨釘在前牙區較易造成潰瘍及不適。本病例以迷你骨釘植入前牙區根尖下區域並改用關閉式術式，並延伸一段以.012英吋的結紮線彎成的掛勾，以彈性鏈圈(elastic chain)連結牙弓弧線(archwire)與結紮線彎成的掛勾，以提供門牙壓入(intrusion)所需之力量，結果在劉固月內即可有效改善深咬。本病例報告將探討植入部位與術式的修改。

Mini-implant anchorage is applied in many clinical orthodontic conditions successfully, including maximal retraction in protrusion cases, Class Ⅱ correction, Class Ⅲ correction, molar distalization in crowding cases, molar intrusion in molar elongation cases, deepbite correction, midline correction, and the correction of occlusal plane canting and posterior crossbite. The most common use of mini-implant anchorage is for maximal retraction of anterior teeth. Deep bite correction with mini-implant is not so common because of soft tissue ulceration and patient discomfort.
In this case report, the author put a miniscrew in the subapical area of Anterior teeth with closed technique and extend a hook with .012” ligature wire. Intrusive force for incisors was applied with elastic chains from the archwire to the extension hook. The deep overbite was corrected in three months effectively. The selection of implant sites and the refinement of insertion technique will be discussed.
