白色念珠菌在二種暫時彈性義齒基底材料上的生長－臨床研究

摘要

在義齒性口炎(Denture Stomatitis)的相關文獻發現，彈性義齒基底比硬壓克力樹脂的基底更容易有牙菌斑堆積，易有微生物的孳生，其中以白色念珠菌(Candida Albicans)為最多。此種微生物亦被懷疑為義齒性口炎的主要加助因子(contributing factors)之一。
本研究以兩種臨床上用來做組織調理的彈性義齒基底材料： Visco-gel R (Dentsply，USA)和Soft-liner R(GC，USA) 為研究用的材料，做黴菌培養的活體內試驗(in vivo study)。於同一患者先後使用不同的彈性義齒基底材料，控制不同個體的變因，以比較不同彈性義齒基底材料對義齒產生口炎治療效果的異同，進而建議彈性義齒基底材料的適應症與使用時機。
實驗結果顯示，感染不分菌種真菌的相對風險 (relative risk)上，Soft-liner R於第四週時較高，相對的在二十四小時、第一週、第二週的風險略低；而感染白色念珠菌的相對風險上，Visco-gel R於第四週較高，相對的在二十四小時、第一週、第二週的風險略低。不分菌種真菌及白色念珠菌的感染，都在第一週時最低、第四週時最高。因此若以真菌感染情形來考量，應盡量以配戴一個星期為限。由於本次的活體內試驗之樣本有限，需要再將樣本數提高，以增加統計上的信效度 (reliability and validity) ；此外採樣時間點如果再增加也許可以提供更多的訊息。這類問題需要更進一步研究來驗證。

From review of literatures about denture stomatitis, soft denture base liners encourage plaque accumulation and microorganism growth, especially Candida Albicans. It was suggested as one of the major contributing factors of denture stomatitis.
Two soft denture base lining materials for clinical tissue treatment were involved in the study: Visco-gel R (Dentsply, USA) and Soft-liner R (GC, USA) Fungus culture was performed on these two materials, and it was an in vivo study. The study was proceeded with a cross-over design, using two materials on the same samples during two treatment periods, controlling for individual variation. The purpose was to compare the treatment effects of the two materials on denture stomatitis, and to identify the indication and timing of denture base lining materials.
The result was that the relative risk of fungus infection was high in Soft-liner R at the fourth week. In the contrast, at the 24th hour, the first week, and the second week, the relative risk was low. About Candida Albicans infection the relative risk of Visco-gel R at the fourth week was high, but at the other time points, the relative risks of Visco-gel R were low.
Both fungus infection and Candida Albicans infection were on the lowest level at the first week and on the highest level at the fourth week. Considering fungus infection, soft denture base lining material should not be used more than a week.
