協助一位青少女懷孕面臨胎死腹中的護理經驗

摘要

由於青少女的身心尚未發育成熟，從懷孕至生產的過程，會出現許多生理、心理與社會問題，給青少女、其家庭與社會帶來沉重的壓力；本篇報告爲描述一位青少女因和家庭關係不和諧，逃家在外與男友同居，於妊娠第三期面臨胎死腹中的護理經驗。筆者爲個案主護護士，護理期間爲94年9月21日至94年9月23日，運用紐曼系統理論的評估，並和個案建立良好人際關係，藉由觀察、會談、臨床照護及出院後電訪，依生理、心理、社會文化、發展與靈性領域範圍收集資料；資料分析後確立主要護理問題包括：預期性哀傷、焦慮、知識缺失等三項。藉由與個案共同討論各項護理問題，以提供整體性及個別性的照護，期望透過此護理過程，協助個案渡過傷慟及胎兒死產的低潮，及早重返社會和父母恢復溝通。

A teenager whose physical and psychological development was immture, as she got pregnancy which would bring a lot of pressure to herself, family and society. This article was to describe an unmarried teenager who fled from her family to live with boyfriend and got pregnancy in the third trimester with an intrauterine fetal death. The writer was a primary nurse during her hospitalization from September 21 to September 23,2005. With establishing a warm patient-nurse relationship, the data were collected by observation intervention, face to face interview, physical assessment, and post-discharge telephone interview. Applying Neuman theory and classified into five categories: physiology, psychology, socioculture, development and spirituality. Three nursing problems included: anticipant grief, anxiety and knowledge deficit. The writer discussed with the case and provided a holistic, individualized nursing care to help her to go through the grief of fetal death, re-establish a harmonious relationship with parents and finally return to society.
