協助一位未婚青少女照護早產兒之護理經驗

本文是在照護一位16歲少女未婚懷孕並早產娩出一29週早產兒之護理經驗，筆者在護理期間發現個案因未婚生子遭受家人指責、批評，面對早產兒的茫然無助、獨自面對父母的生氣與失望以及家庭運作出現危機。護理期間為2012年6月9日至2012年8月12日，故於個案住院及到院探視案子時採觀察、會談及電話訪談等方式收集資料，運用羅氏適應、模式進行個案評估及APGAR家庭功能評估表進行家庭功能評估，個案存在著以下三個護理問題：1.潛在危險性父母；2.家庭運作過程改變；3.照顧者角色緊張。個案由未婚產子面對家人的指責到取得諒解進而全家共同參與照顧，筆者以家庭為中心的整體性護理，協助個案成功化解家庭危機，並建立照護早產兒的責任與能力，希望藉此護理經驗能提供護理同仁照顧此類個案之參考。

This case study described the nursing care experience of looking after an unmarried pregnant 16 years old young girl who delivered an 29 weeks premature baby. The patient was blame and criticism of her family, faced with preterm children very ignorant and helpless. Her parents felt angry and disappointed with her. Her family's support system developed an urgent crisis. The nursing care was from June 9, to August 12, 2012, Over the nursing period, the author applied the skills of observation, conversation and telephone interviews to collect data and also used Roy’s adaptation model and functional assessment table APGAR family home functional assessment to assess and improve the patient's health issues. Identified the case major health problems including: 1.risk for parent; 2.interrupted family processes; 3.caregiver role strain. Case by unmarried birth families face accusations to get an understanding and then take care of the whole family to participate. The author use family-centered holistic care to rebuild the family operation and solved the crisis, and establish child care responsibilities and ability of preterm. By this nursing experience provide practical example for nursing staffs to reference in the care of these patients.
