This article described the nursing process of assisting a case and her husband to face the anxiety from giving birth to a premature infant, and to prepare discharge with continuous oxygen support for the infant. The nursing period was from December 22, 2010 to January 14, 2011. The author used family assessment to identify the problems, which included: lack of knowledge, caregiver role strain and altered family operational processes. Through a series of hospital discharge preparation, including the discussion of medical team, active care and the provision of social resources, the case and her husband adapted to the situation and the premature infant was successfully discharged and received continuous care at home. This case report can serve as a reference for fellow nursing colleagues on discharge planning for premature infants, which would ensure a holistic, perioperative care for the infants involving all family members.
