運用家庭評估模式於一位早產兒合併水腦之護理經驗

A Nursing experience of a Preterm Infant with Hydrocephalus by the Family Assessment Model

摘要


本文旨在描述運用家庭評估模式於一位早產兒合併水腦之護理經驗,護理期間自2014年06月05日至2014年06月28日,筆者藉由觀察、身體評估、直接護理、會談、查閱病歷等方式進行資料收集,依身、心、靈、社會及家庭功能進行整體性評估歸納分析,確立個案與家庭健康問題為:顱內調適能力降低、急性疼痛、照顧者角色緊張、家庭因應能力失能,依個別性照護需求,緩解個案急性健康問題,以家庭為中心之護理,運用家庭評估模式,深入探討家庭所面臨的困境,藉由家庭會議的召開,建立家庭成員共識,共同擬定學習計畫,藉由不斷教導與回覆示教,增進照護者返家照護能力,與家庭因應能力,提升其返家後生活品質。

This paper intended to depict the nursing experiences of a premature infant complicated with hydrocephalus by applying the family assessment model. From June 5 to June 28, 2014, the author collected information by observation, physical assessment, direct nursing, communication, and medical records. Assessment and analysis were conducted holistically according to the physical, mental, spiritual, social and familial functions. We determined the problems between case and family health included reduced intracranial accommodative capacity, acute pain, caregiver role strain, and ineffective family coping ability. The acute problem was relieved and family-centered nursing care was provided according to personal need. By applying this family assessment model, family dilemma was assessed. Through family meetings, family consensus was established, and educational plans were made jointly. The family members were taught constantly and demonstrated themselves as being capable of correct action so as to enhance the caregiver nursing capacity and family coping ability as well as to improve the quality of life after discharge.
