摘要

本文旨探討協助患有周腦室白質軟化症早產兒返家護理經驗，護理期間自2018年5月7日至6月15日，在案父母均無早產兒照顧經驗情形下，經由觀察、家屬會談、病歷查閱方式收集相關資料，並以系統身體評估及個案家庭功能評估。確立主要之護理問題有「無效性嬰兒哺餵方式／與早產、口咽肌肉不協調有關」、「身體活動功能障礙／肌肉骨骼功能障礙」、「照顧者角色緊張／無法預測的照顧情境」三項護理問題，故擬定具個別性護理計畫，給予完善且整體性的照顧，指導案父母學習早產兒返家後照護知識、口腔及四肢復健治療與諮詢管道，進而提升主要照顧者在照顧上的能力與信心，使個案得以順利出院返家，並能獲得持續性的照護。

This study aims to explore the experience of assisting in caring for premature infants with periventricular leukomalacia after returning home. The care period was from May 7, 2018 to June 15, 2018. The parents participating in this study did not have prior experience of caring for premature infants. Issues with care were identified through data collection (e.g., observation, family interviews, and reviewing patient history records). After systematic physical assessment and evaluating the family functions of individual cases, confirmed care issues include invalid feeding methods towards infants/related to premature birth and uncoordinated oropharyngeal muscles; physical activity/musculoskeletal dysfunctions; and nervousness of the caretaker role/unpredictable care scenarios. Thus, personalized care plans were established to provide comprehensive overall care by guiding parents to learn the care information required once the premature infant return home. In addition, consultation channels for mouth and limb rehabilitation therapy were also established. This increased the ability and confidence of primary caregivers to allow individual cases to be successfully discharged and returns home to receive continued care.
