協助一位初次腦中風病人復健之護理經驗

摘要

腦血管疾病為國人第三大死因，其嚴重性影響甚遠。本文探討照護一位腦中風導致右側肢體癱瘓病人的復健經驗。護理期間2010年8月14日至2010年9月14日，應用Gordon十一項功能性健康形態評估為工具，將照護過程中個案所表達的語言與非語言行為加以記錄、整理、分析及歸類，確立個案護理問題有身體功能性障礙、穿著自我照顧能力缺失以及無力感。護理過程透過關懷、同理心、鼓勵表達內心感受、耐心傾聽等技巧，並且提供適切性的護理指導，鼓勵個案及其家屬能面對護理問題，進而順利渡過復健治療過程，促使盡早達成疾病的自我管理能力。藉此提出護理經驗，期望於專業領域中做經驗分享，以提供日後護理人員照護腦中風病人復健的護理參考。

The cerebrovascular diseases are the third leading cause of deaths in Taiwan, indicating its major impact on the public health in Taiwan. This article probes the nursing experience of caring for a patient with right sided paralysis secondary to brain stroke. The nursing period is from August 14 to September 14, 2010, while the patient received post-stroke rehabilitation. With the application of Gordon's 11 functional health patterns' assessment, through observation, recording, and analysis, the major nursing problems identified were neuromuscular dysfunction, inability of self-management for daily activities, and powerlessness. Nursing interventions include encouraging the patient to express internal feelings with nurse's caring and empathetic support and active listening. The patient and family were also encouraged to face the issues encountered, go through the rehabilitative process with guidance, and further achieve the goal of self-management. The author expects this article can be used as a reference for nurses who care for the patients with cerebrovascular diseases.
