降低加護病房中心導管相關血流感染率專案

A Project to Decrease Central Line-Associated Bloodstream Infection in ICU

摘要


院內感染為醫療照護重要的品質指標之一,且住院期間若因血流感染將會增加醫療費用的支出及延長病人的住院天數,嚴重感染甚至造成病人的死亡。因本院加護病房中心導管相關血流感染率均高於國內外數據,故引發筆者團隊執行此專案的動機,希望藉由專案改善來降低加護病房中心導管相關血流感染率。改善過程中經由相關規範修訂,改善醫師執行導管置入程序,完整率由改善前的69%提升至100%,護理人員執行導管照護程序完整率由53%提升至100%,進而達成降低加護病房中心導管相關血流感染率之目標,以促進醫療品質,提升病人之安全。

Hospital acquired infection is a vital indicator of care quality. Blood stream infections during hospitalization increase medical costs and length of hospital stays. Severe infection can cause patient mortality. An improvement project was designed as a result of higher central line associated bloodstream infection rates in the author's hospital, as compared to other hospitals. The policies and procedures for installation of the center line were revised. Competency of performance improved from 69% to 100%. With revised policies and procedures for central line care, the competency of nursing care improved from 53% to 100%. The aim of improving care quality and patient safety has been achieved.
