本專案旨在提升手術室護理人員術前訪視執行率,以提升手術全期護理服務品質。術前訪視執行率低的問題為:需術前訪視手術排程截取錯誤或醫師輸入手術排程太晚、缺乏手術前訪視相關護理照護知識,擔心專業能力不足,無法回答病人及家屬所提的問題、缺乏術前訪視教育訓練、缺乏適當的排班、稽核制度。經由決策矩陣分析後,以修訂與重整各科術前訪視收案條件、依手術代碼更改資訊系統、固定手術前訪視流程規範和宣導、規劃稽核制度和品管監控計畫、規劃訪視案例分析經驗分享課程等方法,將術前訪視執行率由82%提升至94%,顯示藉由教育訓練宣導及修改需訪視個案的手術排程資訊化、配合發理人員需求排班、醫護溝通協調合作,能有效提升術前訪視執行率,提高手術照護品質。
The aim of this nursing project was to increase the rate of operational room nurses' prebetween operational patient contact (POPC) to improve their care and quality of service. The problems causing a low POPC rate included POPC schedule errors, doctors' late submission of operational schedules, insufficient care knowledge to answer those questions that patients and families might bring up during POPC, lack of POPC educational training, improper operational scheduling, and lack of a monitoring system. With analysis using a decision matrix, the adopted improving methods included revising and restructuring POPC criteria for case intake, changing the informational system in accordance with surgery classification codes, regulating and promoting POPC workflow, designing a monitoring system and a quality control plan, and developing the courses for case study and experience sharing. The implementation of these strategies improved the POPC rate from 82% to 94%. This project indicates that the POPC rate can be effectively raised for operational care quality through staff educational outreach, POPC schedule modification via an informational system, nursing needs-matched scheduling, and collaborative coordination between doctors and nurses.
