摘要

本文探討一位初次腦中風入院後復發腦梗塞個案之護理經驗。照護期間自2019年3月26日至4月11日，筆者以觀察、會談、身體評估及病歷查閱等方式收集資料，並運用Gordon十一項功能性健康型態評估，確認個案主要之健康問題有：知識缺失、身體活動功能障礙、自我照顧能力缺失、吞嚥障礙及焦慮。筆者藉跨領域團隊合作、與個案及家屬共同擬訂復健及吞嚥訓練計畫，改善個案身體活動功能、自我照顧能力及吞嚥、進食情況；透過提供對疾病的瞭解、預防再次腦中風相關知識，緩解個案焦慮。期盼此護理經驗能提供日後臨床照護參考。

The purpose of this article was to explore a nursing experience for a first time stroke patient with recurrent cerebrovascular accident. From March 26 to April 11, 2019, data were collected through observation, interview, physical assessment, and chart review according to Gordon' 11 functional health patterns. The major nursing problems identified included deficient knowledge, impaired physical mobility, self-care deficit, impaired swallowing, and anxiety. Through cross-disciplinary teamwork and developing rehabilitation and swallow training programs, the author improved the physical mobility, self-care, and swallowing/eating function of the patient. By providing knowledge of stroke and recurrent cerebrovascular accident, the author relieved the anxiety of the patient. Hopefully, this nursing experience can serve as a reference for the care of such patients.
