醫療從業人員對醫病溝通會議之評價

The Evaluation of Medical Communication Meeting from the Medical Practitioners

摘要


醫病溝通會議是一種醫護人員與病人或家屬溝通最有效之方式,使醫病之間能達成共同決策及共識,而醫療從業人員對醫病溝通會議的態度可以決定行為意向,進而決定行為。本研究目的為探討臨床上曾參與醫病溝通會議之醫療從業人員對醫病溝通會議之態度與行為,及其對召開醫病溝通會議之困境與成效,並比較不同職類醫療從業人員對醫病溝通會議之評價。採用橫斷式研究設計,以臺北市某區域教學醫院為收案地點,針對醫療從業人員,包括醫師、護理人員、社工人員等為研究對象,獲得253份問卷。研究結果顯示,醫療從業人員對醫病溝通會議抱持正向態度、且其行為具有正向趨向,而對召開會議顯示無趨向困境之自覺,又對困境的因應偏屬正向。藉本研究瞭解醫療從業人員參與醫病溝通會議之困境,以及對於醫病溝通之相關態度與行為,有助於發展醫病互動模式,並提高醫病溝通之品質面與滿意度。

Medical communication meetings provide the most effective method for discussion between physicians and medical personnel, patients or their families. Decisions and consensuses between physicians and medical personnels, and patients can be reached by this type of discussion. The aspect of physicians and medical personnel towards medical meetings can determine objective and hence conduct. This study aims to explore the effect of physicians and medical personnels' attitudes and behaviors towards the medical communication meetings; to explore their distress at medical communication meetings conducted during hospital admissions; and to also compare the evaluations of physicians and medical personnel s from different specialties. This was a cross-sectional study conducted at a regional teaching hospital in Taipei. A total of 253 questionnaires were obtained from conducting survey focused on physicians, nurses, and social workers. The results showed physicians and medical personnels have positive attitudes and behaviors toward medical communication meetings, and they tended to feel less distress at medical communication meetings. Recognition of physicians and medical personnels' distress at medical communication meetings, as well as their attitudes and behavior toward patient communication, is expected to facilitate the development of practitioner versus patient interaction model. Furthermore, it improves the quality and satisfaction between patient and physicians and medical personnels' communication.
