一位初診斷愛滋病併巨細胞視網膜炎個案之照護經驗

Care Experience of a Case with Newly Diagnosed AIDS and Giant Cell Retinitis

摘要

本文描述一位中年男性，因視力模糊求治後確診愛滋病，病人除了需面臨巨細胞視網膜炎可能導致失明風險外，更擔心疾病曝光後影響家人間的感情，故拒絕家屬知悉病情；照護期間自2018年3月28日至4月14日，經由Gordon十一項健康功能模式為評估工具，以身體評估、實際照護、會談等方式，確立個案有感覺知覺改變、活動無耐力、體溫過高及焦慮之健康問題，照護過程中除教導健康相關知識外，與個案建立治療性及信任的護病關係、鼓勵病人表達內心感受，運用護理過程，協助病人舒緩負向情緒，使病人對疾病有正向調適技巧，增加病人對疾病認知及自我照護能力，進而獲得良好生活品質，筆者期望藉由護理經驗分享，提供臨床護理人員未來在照顧此類病人時，能主動給予適當協助。

This article describes a middle-aged man who was diagnosed with AIDS initiated from seeking medical advice of blurred vision. In addition to face the risk of blindness caused by giant cell retinitis, the patient was more concerned about the impact of the illness on family members' relationship, so he refused to reveal the condition to his family. From March 28 to April 14, 2017, through Gordon's Eleven Health Function Model as an assessment tool, physical assessment, actual care, and interviews were applied to establish the concerns of the patient with sensory perception changes, activity intolerance, and high temperature. In resolving anxiety health problems, in addition to teaching health-related knowledge during the care process, a therapeutic and trusting relationship was developed with the patient, encouraging him to express his inner feelings, and applying the nursing procedure to help relieve negative emotions. Development of positive adjustment skills enabled patient's awareness of disease and self-care ability to obtain a good quality of life. The author hopes that through sharing this nursing experience, clinical nurses will be able to actively provide appropriate assistance when caring similar patients in the future.
