降低呼吸照護病房病人壓傷發生率

本專案旨在改善呼吸照護病房病患壓傷發生率，專案動機係因本病房病患壓傷發生率逐月上升，2018年12月壓傷發生率1.5%、2019年2月4.37%高於1月3.65%。專案改善過程中藉由擬訂壓傷預防月、舉辦教育訓練及製作防壓傷海報、專人輔導情境式教學及翻身體驗營、訂定翻身擺位標準流程、設計翻身流程趣味化標語、增設輔具用物、修訂完整壓傷稽核制度。實施後壓傷發生率2019年9月至11月為0.15%、0.16%、0.15%，皆低於目標設定值0.5%，本專案實施經驗及成果，對未來照護上建議單位持續執行壓傷在職教育課程，增加主要照護人員對壓傷分級認知與預防照護，以預防壓傷發生，藉以提升照護品質。

The purpose of this project is to improve the incidence of pressure injuries among patients in the Respiratory Care Wards (RCW). The motivation for the project owed to the incidence of pressure injury in this ward escalated for some months. The incidence of pressure injury in December 2018 was 1.5%, and in February 2019 became 4.37% which was higher again than 3.65% in January. In the process of project improvement, strategies developed include pressure injury prevention month; holding training and producing pressure injury prevention posters; dedicated person to guide situational teaching and rollover experiencing camps; formulating a standard operation procedure of rollover positioning; design interesting slogans for rollover process; designing fun rollover slogans; adding auxiliary equipment; and revising the complete pressure injury audit system. After the implementation, the incidence of pressure injury between September and November 2019 dropped down to 0.15%, 0.16%, and 0.15%, which were all lower than the target setting value of 0.5%. This implementation suggests that the RCW requires consistent on-job training for pressure injury in the future. It is necessary to improve main care personnel's awareness about pressure injury classification, preventive care in order to prevent the incidents of pressure injury, and hence improve the quality of care.
