手足症候群是化學藥物引起的副作用,完整的手足症候群照護可預防及減少病人手足症候群的發生,本專案目的是在提升婦科病房護理人員對婦癌化學治療病人手足症候群護理指導的完整率;於2019年8月1日至10月30日,評估護理同仁執行病人手足症侯群之照護指導完整率僅51.4%,經分析原因後發現:缺乏標準的照護規範、護理人員的認知不足、護理指導工具不完善、缺乏稽核制度等。本專案之執行時間自2019年11月1日至2020年4月30日,改善策略有:修訂手足症候群照護規範、在職教育及測驗、製作指導教材及影片、確立考核機制等;專案後,護理同仁在照護指導的完整率提升到95.7%。本專案所研擬的手足症候群照護指導教材,建議做成二維條碼的掃瞄辨識,以嘉惠臨床醫事人員及癌症病人使用。
Hand-foot syndrome (HFS) is one of the common dermatological side effects, which was associated with many chemotherapeutic agents. Comprehensive nursing care can reduce the risk of HFS and alleviate symptoms when HFS occurred. The purpose of this project is to improve the completion rate of nursing guidance in chemotherapeutic patients at risk of having HFS. Before the improvement program, from the first of August to the thirtieth of October in 2019, the completion rate was merely 51.4%. After analyzing the current status, the followings were discovered: lack of standard practice, insufficient cognition of nursing staff, ineffective guidance tools, and lack of auditing system and so on. The implementation period of this project is from the first of November to the thirtieth of April in 2020. Improvement strategies include: revision of hand-foot syndrome care standards, on-the-job education and testing, production of instructional materials and videos, establishment of an assessment mechanism, and so on. After the improvement program, the completion rate increased up to 95.7%. The hand-foot syndrome nursing guidance materials developed by this project are recommended to be accessible by barcode scan to benefit clinical medical staff and cancer patients.
