跨單位支援可使醫院人力有效運用,亦可增加人員第二專長,但若工作壓力超出負荷,可能造成人力流失,甚至離職率增加。經調查發現,本單位護理人員跨單位支援滿意度僅58.2%,分析主要原因為未有明確的支援相關規範,且見習天數短,人員不了解支援單位病房常規、常備藥物項目及檢查轉送輸入方式與特殊處置,加上病房照護病人人數多,不易掌握病人動向,而在遇到困難時人員亦不知如何反應給主管。透過制定支援作業相關規範、制定支援輪序表、製作胸腔科組支援參閱手冊、製作支援心靈紓壓暨意見箱、舉辦主管與支援人員暢談會及舉辦支援人員歡迎會等方案,使內科加護病房護理人員跨單位支援滿意度提升至87.3%,達成專案改善目標。
Cross-Unit support can allow medical personnel to be effectively utilized, which also enable them to gain a second set of skills. However, if the work pressure exceeds the load, it may lead to the loss of manpower and elevate the rate of staff overturn. According to examination, the satisfaction rate of the nursing staff in our unit performing cross-departmental support is only 58.2%. The analysis discovered the reasons include: ambiguous supporting regulation, short training days resulting in new personnel unfamiliar with ward regulations when the supporting other units; lack of knowledge in common medications and the methods of examination, as well as transfer, input and special measures taken for a patient. In addition, the large number of patients in the wards makes it difficult for medical personnel to have a solid grasp of the movement for all patients; also the medical personnel do not know how to report back to the supervisor when encountering difficulties. Through the implementation of relevant regulations on supportive operations, formulate support rotation schedule, create Thoracic Medicine Division Support Manual, establish a morale support and stress relieve suggestion box, hosting seminars for supervisors and support personnel as well as their welcoming party, the cross-unit support satisfaction rate for Medical Intensive Care Unit has increased up to 87.3%, that achieving the goal of project improvement.
