手術室的運作直接關係病人的治療效果與醫院營運,本單位2021年1至7月收到院方通知有24件病人自費醫療衛材未記帳,漏帳金額高達503,371元,故本專案旨在提升手術室護理人員醫療衛材計價之正確率。經現況分析發現,計價表單未定期更新,表格太密集;護理人員未依照標準作業執行計價,入帳過程被中斷導致電腦入帳錯誤,書寫潦草導致輸入錯誤與不熟悉給付規則;醫療衛材擺放雜亂,計價作業標準簡略及廠商給錯計價碼,造成護理人員醫療衛材計價正確率僅75.2%。經建置醫療衛材計價作業資訊系統,修訂計價流程標準書,辦理在職教育,建立與廠商核對機制與計價提醒機制,及重整衛材室配置等,醫療衛材計價正確率提升至97.2%,漏帳金額降至110,281元。本專案的推行可有效提升護理人員醫療衛材計價正確率,為病人手術安全與醫院成本把關。
The function of the operating room directly affects patient treatment outcomes and hospital performance. Our unit received a notification from the hospital from January to July 2021 stating that there were 24 cases in which patients were charged for medical material, but were not adequately recorded. This resulted in a highly unrecorded charges amount to NT$503,371. Therefore, this project targets to improve the accuracy of nursing staff in the operating room for calculating medical and sanitary materials costs. Based on the current situation analysis, it has been identified that the material costing sheet has not been updated regularly, and the tables within the sheets were too dense. The nursing staff failed to follow the standard operation procedure when calculating, and the interrupted charging process results in data entry errors. Additionally, sloppy writing leads to input errors and unfamiliarity with payment rules. The placements of medical and hygiene materials were cluttering, the pricing operation standards were too simplified, and the manufacturer provided incorrect pricing codes. Consequently, the nursing staff's medical and hygiene materials pricing accuracy rate was merely 75.2%. After implementing a medical materials pricing information system, revising the pricing process standard book, conducting on-the-job education, establishing a mechanism for checking with the suppliers, and implementing a pricing reminder mechanism, including restructuring of the medical material room configuration, the medical supply charging accuracy rate has increased to 97.2%. Moreover, the number of unrecorded charges reduced to NT$110,281. The implementation of this project has effectively improved the accuracy of nursing staff in medical material pricing and hence ensures patients safety during surgery while controlling hospital costs.
