運用團隊資源管理提升手術室病人安全

Using Team Resource Management to Improve Patient Safety in the Operating Room

摘要


目的:手術作業程序仰賴多專業團隊成員貢獻,然而,多數不良事件肇因於手術過程的跨領域團隊溝通失效。故本文旨在簡介完整體現團隊資源管理(Team Resource Management, TRM)概念之團隊策略工具提升效能及病人安全(Team Strategies and Tools to Enhance Performance and Patient Safety, TeamSTEPPS)工具,以提供醫療團隊於病人手術照護期間之參考。方法:簡介團隊資源管理之概念及團隊策略工具於手術室之應用。結果:團隊策略工具應用於手術室,除能提升團隊運作相關之知識、態度及技能與效率,並可降低死亡率、手術併發症、手術部位錯誤率、人員失誤及儀器設備短缺等病人安全事件。結論:若能善用工具於排除組織問題並將照護標準化,確實能降低負面結果。然而,建議更應著重安全文化之建立,提升整體遵從性,才能有效防堵病人安全事件再發生。

Objectives: Surgical procedures rely on the contributions of multi-professional team members. However, most adverse events result from a failure in cross-disciplinary team communication during the surgical process. The purpose of this article is to introduce TeamSTEPPS (Team Strategies and Tools to Enhance Performance and Patient Safety) which is fully embodied in the concept of Team Resource Management (TRM), so as to provide a reference for the medical team in the period of patients' surgical care. Methods: To introduce the concept of Team Resource Management and the application in TeamSTEEPS in the operating room. Results: Applying TeamSTEEPS in the operating room to enhance the knowledge, attitude, skills and efficiency of team operations, and also improve patient safety events such as the reductions in mortality, surgical complications, surgical site error rates, staff errors and shortage of equipment, etc. Conclusions: The use of TeamSTEEPS to troubleshoot organizational issues and standardizing care procedures can ultimately reduce negative consequences. However, it is recommended to focus more on the establishment of safety culture and improve overall compliance to effectively prevent the recurrence of patient safety incidents.
