Health Status and Medical Care Needs of the Disabled Elderly: An Experience of Nantou County

摘要

本研究旨在瞭解身心障礙老人健康狀況、醫療利用與醫療服務措施需求，並提出相關長期照護政策建議。樣本：以「南投縣2005年身心障礙者生活狀況與福利需求調查」領有身心障礙手冊之65歲以上老人為對象，共227名。結果：身心障礙老人相較於一般老人或身心障礙者，健康狀況較差，醫療利用也較高，對各項醫療服務措施普遍表達高需求。89.0%患有一項以上本研究界定的疾病；三個月內就醫率87.2%，一年內住院率30.0%。影響醫療利用的因素：罹患高血壓、糖尿病者，有明顯較高門診率；外出行動困難、生活自理能力依賴、罹患心臟病、糖尿病者，有明顯較高住院率。影響醫療服務措施需求的因素：身心障礙發生時期在中老年之後、身心障礙程度較高、罹患心臟病、糖尿病者，個別性需求較高；身心障礙發生時期在中老年之後、身心障礙程度較高者，一般性需求較高。建議：(1)重視身心障礙老人的雙重經驗；(2)以可近性的社區照顧資源舒緩非正式照顧的負擔；(3)強化中老年健康保健教育及慢性病預防與理。

This study aims to understand the physical health status and medical care service utilization and needs of disabled elderly. The research data abstracted from ”The survey of living status and welfare demand of the disabled people, Nantou County 2005”. A total of 227 disabled and aged 65 and over were sampled. The result showed that the disabled elderly are characterized as poor physical health, and with highly medical care service utilization and needs. Most of the subjects reported having at least one kind of illness (89.0%), 87.2% had visited outpatient clinic in past 3 months and 30.0% were hospitalized in past one year. Subjects with hypertension or diabetes had more outpatient visits, those who had difficulty in going out, dependently in daily life activities, suffering from cardiopathy or diabetes had more hospitalizations. Those who became disabled after middle-age, with more severe disabilities, suffering from cardiopathy or diabetes had higher personal care service needs, those who became disabled after middle-aged, with more severe disabilities had higher general care service needs. Researchers propose: (1) taking attention to the dual experiences of disabled elderly, (2) offering accessible community care resources to reduce the burden of informal caregivers, (3) strengthening the middle-aged people healthcare education and chronic diseases prevention and management.
