社區藥事照護與推展－兼論醫療團隊之角色

摘要

用藥安全是醫療品質重要的一環，老人不良就醫習慣、多重用藥或自行用藥等，皆可能造成對健康的危害與不必要的醫療支出。隨人口老化用藥增加，重視社區藥事照護服務成為國內外發展趨勢。本文藉由文獻回顧，闡述社區藥事照護之重要性、醫療團隊在社區藥事照護中的角色與須具備之特質、社區哪些地方優先需要藥事照護介入、及如何成功的推展。結果發現所有醫療專業人員，包括醫師、藥師及照護人員等都應瞭解學習藥物治療過程中正確的專業知識，醫師及照護人員應瞭解病人完整用藥情形及藥物儲存是否合宜。藥師基於其專業，應提升醫護人員對合理用藥的重視，及諮詢藥物相關資訊的管道與機會，間接協助社區中照護人員將病人安全達到最佳結果。建議未來除了持續教育民眾正確用藥觀念外，善用醫療資訊系統、整合現有社區資源與照護體系、加強醫護專業人員與藥師之合作等方式來進行社區藥事照護，並建立具體成效衡量指標，皆是國內未來可努力之方向。

Medication safety is important in quality of health care. Doctor shopping behavior, polypharmacy, and self-medication in the elderly are harmful to health and cause healthcare waste. It has become an international trend to provide community pharmaceutical care to improve medication safety, because medication use are increasing with population aging. We reviewed the literature to illustrate the importance of community pharmaceutical care, the role and personality of the pharmacists, what place in community need pharmaceutical care first, and how to promote it successfully. We found that all healthcare professionals, including physician, pharmacists, and healthcare providers, should understand the knowledge of medication therapy. Physician and healthcare providers should know medication history of the patients and whether they store the medication properly. Pharmacists should let healthcare providers put more emphasis on proper medication and provide them the way and opportunity to consult the information of medication, and help them care patients to the best results. What could be done in the future are as follows: educate people proper medication use, make good use of healthcare information system, integrate existing community resources and healthcare system, increase cooperation of healthcare professionals and pharmacists, and establish specific indicators to evaluate the effects.
