摘要


以個案為中心的照護,需要專業間的整合,專業間合作成為重要議題。本文介紹長期照護合作常見的三種模式及其優缺點:多元模式、互動模式及跨專業模式,再說明居家營養專業合作過程。隨著長期照護需求的增加,專業人員照護服務角色越來越重要,專業間合作宜更加強。

Inter-professional collaboration is the cornerstone of successful integrated care in long-term care. Three kinds of professional collaboration models and their advantages and drawbacks were introduced in this paper. In the meantime, how to operate home nutritional service in Taiwan was also illustrated in the paper. It is imperative to further promote inter-professional collaboration with the increase of long term care needs and gradually the predominant roles of professions.
