本研究目的主要在探討國軍醫院員工對於目前所服務醫院現況之滿意度,以及各個經營體質改善方案之看法與未來意向,以做為未來國軍醫院經營體質改善之參考。研究以問卷調查的方式進行,針對全國18家國軍醫院之正式員工(含軍職及軍、民聘僱)進行全面性之普查,對象共計7,277人。調查時間係89年7月10日至89年8月31日,共回收3,339份有效問卷,回收率為45.88%。調查之經營體質改善方案包括基金與預算改制、首長經營、半開放制、設立經營法人、委託經營與成立法人等六個模式。研究結果顯示,員工除了人際溝通面向外,普遍對醫院現況感到不滿意,而在各改革方案之比較中,半開放制度、設立經營法人、委託經營、成立法人等四個方案獲得國軍醫院員工普遍的支持與認同,認為在這些制度下醫院的經營將得以改善,亦有較高的留任意願,而對於改革幅度有限的基金與預算改制則最不青睞。因此,未來國軍醫院若欲從事經營體質改革,應可推動相當幅度之變革,不僅可獲取員工方面相當程度的認同,亦可獲得經營體制上相對的彈性與機制,將更能面對日益競爭的醫療環境。
This study attempted to assess the satisfaction level of military hospital employees with their workplace, their opinions regarding different restructuring proposals and their desire to continue to work in the military hospital system, with the aim of providing a reference for future reform of military hospital operating models. This study was based on a census survey sent to 7,277 permanent staff members of the 18 military hospitals in Taiwan (including current military personnel and as contracted employees). The survey was performed between July 10, 2000 and August 31, 2000 and 3,339 valid responses were obtained for a response rate of 45.88%. Opinions were sought on the six proposed operating models: (1) current budget system reform; (2) executive responsibility; (3) semi-open hospital; (4) non-profit management foundation; (5) contracting-out; and (6) non-profit organization. The analytical results showed that except for employee communications, most respondents were dissatisfied with current hospital condition. Out of the six proposals, semi-open, non-profit management foundation, contracting-out, and non-profit organizations received popular support and approval, and most employees believe that these four proposals would improve hospital operations. The respondents considered the limited changes inherent in the budget system reform to be least attractive. Therefore, the military hospital system could implement significant restructuring in its operating model, with a high degree of approval by its employees and achieving relatively good operational flexibility and control under a new operating model in an increasingly competitive healthcare environment.
