台灣每年將近三萬人死於癌症,但只有約20%的末期病人接受到安寧緩和醫療服務。聽過安寧緩和醫療的人很多,但真正瞭解的卻極為少數。本研究目的在瞭解某科技大學護理系與醫管系學生對安寧緩和醫療知識之認知及其差異。研究方法以某科技大學之護理系與醫務管理系日間部學生為研究對象,共計取得431份有效問卷。研究工具採自填之結構式問卷。統計方法包括描述性統計、t檢定、單因子變異數分析、Scheffe事後檢定、Mann-Whitney U、Kruskal-wallis test以及複迴歸分析。研究結果為護理系之安寧知識平均得分高於醫管系;女性高於男性;二技高於四技;曾修過「長期照護」課程之醫管系學生高於未修過者;曾在醫院實習者高於未曾實習者;曾參加「安寧緩和醫療」相關講座者高於未曾參加者,以上皆達統計上顯著差異。對於安寧緩和醫療之相關法規、疼痛控制、嗎啡使用方面的知識明顯不足。研究結論為醫療相關院校應加強安寧緩和醫療相關課程及相關之法令,以增進學生對於安寧緩和醫療之正確認知及適當的生死觀,並能正確瞭解我國相關法令與健保制度所能提供之協助,以使需要安寧緩和醫療之病人能夠接受適切的照護。
Background: Approximately thirty thousand cancer deaths occur annually in Taiwan, and only 20% of terminal patients receive hospice palliative care. Although most patients understand the concept of palliative care, few have detailed knowledge. Objectives: This study compared differences in knowledge of hospice palliative care between nursing and healthcare administration students. Methods: The survey employed a structured questionnaire distributed to 431 university nursing and healthcare administration students Statistical methods included descriptive statistics, Student t test, ANOVA, Scheffe post hoc, Mann-Whitney U, Kruskal-wallis test and multiple regression. Results: Nursing students had significantly higher average knowledge scores than healthcare administration students; the scores of female respondents were significantly higher than those of males; the scores of two-year college students were significantly higher than those of four-year college students; scores of healthcare administration students who took a course in long-term care administration were significantly higher than those who did not take such a course; scores of students who had served as interns were significantly higher than those who had not; scores of students with hospice palliative care training were significantly higher than students without such training. Generally, knowledge was inadequate in hospice palliative care regulations, pain control and morphine use. Conclusion: To ensure the accurate delivery of training, medical universities should provide instruction in hospice palliative care. Nursing students can help optimize hospice palliative care by learning to assist terminal patients under the current regulations and NHI system.
