健康促進醫院組織診斷之個案研究

Organizational Diagnosis of Health Promoting Hospitals

摘要


本研究採個案研究法,以開放系統模式及健康促進醫院自我評估工具來診斷個案醫院推行健康促進醫院之效能與適切性。蒐集資料方法以文件資料蒐集及問卷調查爲主。文件資料蒐集採用本研究設計的「健康促進組織診斷資料蒐集表」進行;問卷調查工具則是WHO歐洲分部所發展之「醫院健康促進標準之自我評估工具」,以個案醫院之院長、科室之主管及相關業務負責人員爲調查對象。 研究結果發現,有關健康促進之效能診斷方面,個案醫院之優點多於缺點,但有部份關鍵性的問題確實存在,包括個案醫院健康促進的總體目標,尚未有明確的短、中、長程目標的設定與相關計畫的擬定,而多數健康促進方案並未進行評估。此外,在作業上則缺乏健康促進方案標準程序執行,且健康促進相關之空間與硬體設備不足。而有關健康促進之適切性診斷,其標準化得分爲75.15,顯示個案醫院健康促進醫院標準的整體達成率屬中上程度。而在五大評估標準方面,以標準化得分來做比較,發現以「促進健康的工作場所」得分爲77.47爲最高,而「病人評估」得分爲65.31爲最低。 本研究建議個案醫院應將健康促進的觀念更具體的描述在醫院的短、中及長期目標當中,使得健康促進的推動能夠內化成爲醫院組織文化的元素之一。本研究亦建議國內需要發展一套本土化的健康促進醫院組織診斷模式、標準與工具,使組織診斷的進行有所依循。

This study assessed the effectiveness and appropriateness of one case hospital implementing the Health Promoting Hospitals (HPH) with the open-system model and self-assessment tool of the HPH. Data were collected from both written documentation and a questionnaire survey. The former was conducted through the 「Health Promoting Organizational Diagnosis Data Collection List」, and the latter used the 「The Self-Assessment Tool for Health Promotion Standard in Hospital」originally developed by the WHO Regional office in Europe. The study produced several key findings. Practical goals and objectives regarding health promotion in the case hospital were inadequate. Most health promotion initiatives did not conduct evaluation. Written procedures for implementing health promotion activities were unavailable. Furthermore, it was suggested that shortage of space and facilities facilitate health promotion programs. Regarding the appropriateness of health promotion, the area of 「creating a healthy workplace」 (standardized score = 77.47) was highly acknowledged. However, 「patient evaluation」 (standardized score = 65.31) revealed a need for improvement. The study results suggest the need for the case hospital to establish comprehensible goals and objectives of health promotion programs and to foster a hospital culture which embraces the value of health promotion. Additionally, development of a model, standards and tools of health-promoting organizations specific to Taiwan hospitals is highly recommended.
