醫療照護技術不斷提升,過度的醫療介入使得長者生命被無意義的延長卻缺乏生活品質,如何讓長者活得久、活得好、活得有尊嚴,更顯重要。本研究旨在探討衛教介入對社區長者預立醫療照護諮商知識與態度之成效。採實驗性研究設計,以雙北市八家社區機構長者為對象,採群落隨機分派方式,將其分為實驗組與控制組。實驗組接受預立醫療照護諮商衛教課程介入,共兩次,每次90分鐘,控制組維持日常照護。以獨立樣本t檢定檢測衛教介入對不同組別長者知識與態度改變量之差異分析。結果顯示,共61位長者參與研究,包含實驗組28位,控制組33位。不同組別長者知識得分改變量分析無顯著差異(t=0.374,p=0.710),在態度得分改變量分析無顯著差異(t=1.919,p=0.059)。本研究提供醫護人員及家屬了解長者對預立醫療照護諮商之情形及衛教介入對長者預立醫療照護諮商知識與態度改善成效,可作為未來設計長者預立醫療照護諮商知識與態度之措施參考。
As medical care technology continues to improve, excessive medical intervention makes the life of the elderly meaninglessly prolonged but lacks the quality of life. Therefore, how to make the elderly live longer, better, and with dignity is very important. This study aims to explore the effect of health education intervention on the knowledge and attitudes of Advance Care Planning for the elderly in the community. This is an experimental research design. The elderly in eight community institutions in Taipei and New Taipei city were recruited. Clustered randomization controlled trial was adopted. The experimental group received health education intervention, each session lasted for 90 minutes, and a total of two sessions were conducted for the experimental group, while the control group continued with its usual care. Independent samples t-tests were used to analyze the differences in knowledge and attitude changes between the two groups. The results showed that a total of 61 subjects participated in the study, including 28 in the experimental group and 33 in the control group. There was no significant difference in the knowledge score change and in the attitude score change (t=1.919, p=0.059) between the two groups (t=0.374, p=0.710). This study provides healthcare professionals and family members with a better understanding of older adults' perspectives on advance care planning and the effectiveness of health education interventions in improving their knowledge and attitudes towards advance care planning. These findings can serve as a reference for future measures aiming at improving older adults' knowledge and attitudes towards advance care planning.
