摘要


肩關節動作分析將有助於臨床工作者診斷及動作功能評估。三度空間肩關節動作包括胸骨、鎖骨、肩胛骨、及肱骨連鎖動作。文獻中有數種方法量測肩關節動作,其中兩度空間研究容易有投影誤差,三度空間分析較為準確。而測量肩關節三度空間動作儀器也有數種。一般而言肩關節動態動作可由具有高可信度電磁儀器測量,近來有許多研究應用此電磁儀器測量方法於中風病人、冰凍肩病人、及夾擠症病人,分析病人肩關節動作病變,並且發現具有臨床意義結果。未來研究應利用電磁儀器測量功能性活動中肩關節動作變化。量化功能性活動中肩關節動作可以加強瞭解病人生物力學。另一方面,分析肩關節動作時必須注意幾個觀念及情況,包括骨位置、追蹤感應器、旋轉角度順序、及避免積伯情況。(物理治療2003;28(6):341-351)

Analyzing shoulder complex movements is an important concern for a clinician in diagnostic and functional evaluation of patients with shoulder disorder. Shoulder complex movements are three dimensional and include chain-related motions of thorax, clavicle, scapula, and humerus. Researchers have suggested several protocols for a quantitative assessment shoulder complex motions. Two-dimensional (2-D) recordings are very sensitive to projection errors, whereas three-dimensional (3-D) motion recording is more accurate. Several motion analysis systems are available for measuring 3-D motions of shoulder complex. Generally the dynamic movements of the shoulder complex can be measured reliably by an electromagnetic tracking device using a standard protocol. Recently, this method has been used to identify hypothesized shoulder complex kinematics theories during simple arm elevations in patients with stroke, frozen shoulder syndrome, and impingement syndrome. More research is needed to investigate shoulder complex motions during functional activities in patients with other types of shoulder disorders. Quantifying the dynamic movements of shoulder complex during functional activities will further enhance our understanding of movement biomechanics in patients. When measuring shoulder complex motions during functional activities, several concepts and conditions, i.e., choosing proper bony landmarks and tracking sensors, identifying rotation sequence, and avoiding gimbal lock condition, should be considered.
