Reliability of Shoulder Complex Movements and Electromyograply during Functional Activities-A Preliminary Study

功能性活動中肩關節動作及肌電圖之信度研究-先驅研究報告

摘要


目的:本研究的目的是測量功能性活動中肩關節三度空間動作及相關肌肉肌電圖之可信度。方法:共徵集10位成年健康男性參與本研究,利用電磁儀器及肌電圖測量受試者慣用手於五種功能性活動(包括手上舉及向前伸手動作等)中肩關節三度空間動作及相關肌肉肌電圖可信度。作者以肩關節三度空間動作(肩胛骨上轉、肩胛骨前傾、肩胛骨外轉、肱骨上舉角度、肱骨上舉平面、及肱骨旋轉角度)及肩關節肌電圖訊號(上斜方肌、下斜方肌、前鋸肌、三角肌、股二頭肌、及股三頭肌)為測試信度之變數。結果:於功能性活動中,由三次測試之變數平均值所得之可信度(0.82-0.99)比單一測試可信度(0.60-0.99)高。另外在測試功能性活動中,測試標準誤差小於2.5度、0.4公分、及3.6百分率肌電圖值。結論:利用電磁儀器及肌電圖測量功能性活動中肩關節三度空間動作及相關肌肉肌電圖,可以得到高信度,特別是利用測試變數之三次平均值。未來研究應測試此種方法運用於肩關節病人之可信度。

Purpose: The present investigation tested the reliability of three-dimensional (3-D) shoulder complex movements and electromyography (EMG) during functional tasks in individuals without shoulder disorders. Methods: The dominant right arms of 10 asymptomatic male subjects (age, 42.0±8.9) were assessed by using a 3-D electromagnetic movement recording system and an EMG system. They performed five functional tasks which involved arm raising and reaching tasks, in a random order. The kinematic variables (peak scapular upward rotation, peak scapular tipping, peak scapular protraction, peak scapular elevation, peak humeral elevation, plane of humeral elevation at peak humeral elevation, and humeral rotation at peak humeral elevation) and the EMG root mean square variables of 6 muscles (the upper trapezius, the lower trapezius, the serratus anterior, the anterior deltoid, the biceps, and the triceps) were the main outcome measure(s). Results: The variables derived from the mean of the 3 trials (ICC=0.82 to 0.99) are more reliable than those from a single measurement (ICC=0.60 to 0.99) to reliably quantify the functional activities for the kinematic and EMG measures. In addition, the standard error of measurement for shoulder complex movements during the functional tasks was less than 2.5°and 0.4 cm. The standard error of the measurement for shoulder muscle EMG during the functional tasks was less than3.6% maximal voluntary contraction. Conclusions: Our method can be used to quantify the shoulder complex kinematics and associated muscular activities during the functional tasks by mean of the 3 trials. Further study should apply this method to subjects with shoulder disorders.
